Back to School Health and Medical Checklist

I hope everyone is having a wonderful summer! We are looking forward to welcoming our students back for another great school year!

As you begin preparing for the first day of school, please take a few minutes to review the attached Back-to-School Health & Medical Checklist. It includes helpful reminders to ensure your child's health needs are in place before school begins.

📋 A few important reminders:
✔️ If your child will need medication administered during the school day, please bring it to the school nurse's office in its original pharmacy-labeled container along with any required medication authorization forms.
✔️ If your child has emergency medications (such as an inhaler, EpiPen, seizure medication, diabetic supplies, or other emergency medications), please provide the medication along with updated physician's orders and emergency action plans so they are available at school if needed.
✔️ If you received a notification before summer break regarding required immunizations, please make every effort to have those completed before the first day of school to avoid any delays in starting the school year.
The attached checklist is a great resource to help you gather medications, update health information, and make sure all required forms are completed before school starts.
